The Better Care Fund is a pooled budget between the CCG and Slough Borough Council to support the integration of health and social care services, so that people can manage their own health and wellbeing, and live independently in their communities for as long as possible. It aims to improve the lives of some of the most vulnerable people in our society, placing them at the centre of their care and support with integrated health and social care support, resulting in improved experience and better quality of life.
2017 sees the third Better Care Fund plan for Slough and marks a mid-point in both our local and the government’s plan for integration of our health and social care services by 2020.
This is the first year that we have produced a two year plan which enables us to plan investments and trajectories for delivery beyond the one-year cycle of our previous plans. Although the scope of this plan is for Slough as a local Wellbeing Board area it is now also firmly set within the context of the newly established Sustainability and Transformation Partnership which will oversee the transformation of health and social care services within and across the Frimley STP footprint as it moves to become an accountable care system supported by new governance and partnership arrangements and shared care records. We will also see the bringing together of the three east of Berkshire CCGs into a single accountable body within the cycle of this two-year plan with a single operating plan for 2018-19.
