As part of the Slough CCG commissioning plan for 2013/14, we have chosen three local priorities that will deliver a measurable improvement within the year of 2013-14.

These local priorities are:


  1. Delivery of nine care processes for people with diabetes (to include BP, cholesterol and blood glucose check) and ensure best practice is delivered to our diabetic population.
    Aiming to achieve at least 67% of all diabetic patients will receive all nine care processes
  2. Improvement in patient satisfaction with their GP practice (as measured by the national GP Patient Survey)
    Aiming to achieve at least 80% satisfaction rate for member practice within Slough CCG
  3. Improvement in patients feeling supported to manage their long-term health condition (as measured by the national GP Patient Survey)
    Aiming to achieve at least 60% improvement within member practices in Slough CCG
